At Hearts & Homes Support Services, we specialise in supporting individuals with Mental Health needs, Learning Disabilities, Autism Spectrum Conditions, and young adults transitioning to more independent living. When intensive rehabilitation is no longer required, but ongoing support is still needed, our Supported Living services offer a safe, empowering next step.
Our flexible, person-centred care packages are tailored to each individual’s needs, promoting greater independence while ensuring they have the support they need to thrive—whether in their own home or in one of our carefully selected supported living properties.
We provide high-quality, small-occupancy accommodation in welcoming residential areas. These home-like settings are designed to foster independence, daily living skills, and personal growth within a nurturing, community-focused environment.
We work with the belief that each person is living in their home—and our role is to support them in building a life that is meaningful, rewarding, and self-directed. Our staff actively encourage involvement in day-to-day decisions, celebrate personal achievements, and support individuals in setting and reaching new goals through positive risk-taking and confidence-building experiences.
Through personalised support, structured routines, and engaging activities, we help individuals live life to the fullest—on their own terms.

Respite Care
Caring for a loved one is a deeply rewarding experience—but it can also be physically and emotionally demanding. Over time, the pressure of being a full-time carer can lead to fatigue, stress, and burnout. That’s where our Respite Care service comes in.

Whether it’s a few hours, a few days, or on a regular schedule, we’re here to provide trusted support when it’s needed most.
Our respite care is tailored to meet the individual needs of both the person receiving care and their family. Our experienced and compassionate staff follow a personalised support plan, ensuring the same high standard of care and consistency your loved one is used to.
Respite care can be arranged in two ways:
Live-in care – One of our staff members stays in the home to provide around-the-clock support and companionship.
Visiting care – For those who need extra help for just a few hours a day or week.
No matter the arrangement, our goal is to offer peace of mind, continuity of care, and a supportive hand when it’s needed most.
Discharge Care
Returning home after a hospital stay can be a vulnerable time—especially without the right support in place. At Hearts & Homes Support Services, our Discharge Care (Hospital to Home) service is designed to ensure a safe, smooth, and supported transition back to familiar surroundings.
We provide tailored Health and Social Care support packages aimed at reducing the risk of hospital re-admission and avoiding the need for temporary stays in residential care. Our approach focuses on helping individuals recover comfortably and confidently at home, with the right level of care in place from day one.
For many, especially those with cognitive conditions, being discharged into a familiar environment can significantly improve overall recovery and wellbeing. That’s why we prioritise timely, coordinated support that meets both immediate and longer-term needs.
Whether care is planned in advance, required at short notice, or needed in an emergency, our experienced staff are ready to step in. We assess the individual’s needs prior to discharge and work closely with families, hospital teams, and other health professionals to put a personalised care plan in place—ensuring a safe return home and ongoing peace of mind.
Day & Night Care
At Hearts & Homes Support Services, our Day & Night Care is designed to support individuals who wish to remain in the comfort and familiarity of their own home while receiving the care they need. Whether it’s assistance with daily household tasks, personal care, or emotional support, we’re here to help every step of the way.
We provide consistent, high-quality care that’s built around each person’s unique needs. Our staff offer more than just practical support—they’re a friendly, reliable presence providing companionship, safety, and reassurance throughout the day and night.
Whether you’re arranging care for a loved one or seeking support yourself, we tailor every care plan to the individual. We work closely with private clients, families, and local authorities to ensure every person receives the compassionate, personalised care they deserve.
Our dedicated team takes the time to match each individual with a member of staff who is not only experienced and highly skilled, but also kind, patient, and genuinely committed to making a positive difference.
Overnight services include:
- Getting dressed and getting undressed
- Assisting you with getting into and out of bed
- Accessing and leaving the bathroom with assistance
- Providing assistance to those unable to move independently
- Management of medication
- Making sure you are comfortable and feel safe
Live-In Care
Our Live-In Care offers a reassuring and dignified alternative to residential care—allowing individuals to remain in the comfort, familiarity, and independence of their own home, with dedicated, full-time support.
Our Live-In Care service provides personalised assistance with day-to-day activities such as personal care, medication, meal preparation, and household tasks. With one of our experienced staff members living in the home, your loved one receives consistent care, companionship, and peace of mind—day and night.
We understand that the thought of moving into a care home can be unsettling. That’s why Live-In Care is such a valued option—it allows individuals to continue living life on their own terms, surrounded by what matters most to them, while receiving the high-quality support they need.
